Well, I'm back. I origianlly started this discussion because I was facing open shoulder surgery in May. Since then, I've had it manipulated under anesth. and another arthroscope in August to clean out scar tissue. During the month of July I had nerve blocks 4x a week and therapy has been everyday except Sundays. My dislocation has been fixed but am caught up in a viscious cycle; it's moving well, scar tissue lays down (even with extensive therapy), pain returns, inflammation returns, and ultimately the whole capsule tightens down so much it hardly moves...thus leading to the scope procedure again. Has anyone had a similar experience? My Dr. says my shoulder is truly the worst he has seen (he's been doing this for 20+ yrs.) and my body produces scar tissue at twice the normal rate. I'm frustrated, tired of the pain, tired of 11 months of therapy, and just want to get back to work. If anyone would like to share, I'd love to hear. Eva

Hi Eva! I've had multidirectional instability of both shoulders for the past 25 years. Had open bankhart and a capsular shift done on my left shoulder last Fall, and here's a success story. Followed rehab protocol faithfully, worked hard at the home exercises (3X/day for about an hour), and am fine now. Have lost only about 5 degrees of ext. rotation. I am a very active person, and am back to all activities (running, biking, swimming, tennis, kayaking, skiing). Post-op was painful for several weeks. Developed some referred pain. Exercises became quite monotonous, but it was worth it. 4 weeks ago I had the same thing done to my right shoulder (dominent arm). So far, recovery is going even better than last time. In fact, I feel like I can do more than I'm supposed to be doing, but being patient through this healing process to avoid reinjury. Highly recommend open surgery - of course, that is because I have a lot of faith in my orthopedic surgeon's ability, experience, and knowledge. Andrew, I have had the open bankhart procedure and a capsular shift done on both shoulders to solve my dislocation/multi-directional instability problem. Total recovery takes 4-6 months. Start with p.t. rehab 2X a week starting a week post-op. P.T lasts 10-12 weeks. Must do the exercises at home - I did them 3X a day, and it usually took an hour each time. The same home program continues after being released from p.t., until 6 months post-op. It's extremely important that you follow the exercise protocol so that you get your range of motion back. External rotation takes the longest to get back. As far as pain meds, I have a high tolerance as well, but really needed the pain med for the first couple of weeks. After that, for a couple weeks, I'd just take one before I went to p.t. to help make sure I got as much motion as possible during therapy. One of my biggest challenges was being able to sleep in bed comfortably - took me 3 months each time! Had one shoulder done in 11/2000, and just had my other shouldr done 10/2001, so I'm still rehabbing that one. Both indicate good stability. It does seem like a long time going through the rehab, but if done diligently, the outcome is worth it!!! Good luck!
